Payment and Liability Agreement for Sittermate LLC
I, _____________________________________ ("Owner"), certify that I am the owner of this pet, and I grant permission to this facility and its staff to obtain on my behalf and in my pets' best interest the veterinary care necessary to treat illness or injury. I agree to pay all veterinary and other necessary services incurred by and for my pet during its stay in this facility. This facility agrees to exercise due and reasonable care to prevent injury or illness to my pet. However, in the event of illness or injury, the facility and its owners and staff shall not be held liable for such injury or illness. I agree to pay a deposit in the amount of 50% of the estimated booking cost prior to the start of services, and I agree to pay the remaining charges the day I pick up my pet. I understand that my pet may not leave the premises until all charges are paid in full. I understand that any animal left for ten days beyond the estimated date of pick up will be considered abandoned.Â
Cancellation Policy for Sittermate LLC
I understand that plans can change, and sometimes cancellations are unavoidable. However, to ensure the fair treatment of all my clients and the efficient operation of my business, I have established the following cancellation policy:
Daycare and Other Reservations:
Cancellation Notice: I require a minimum of 1 days' notice for all cancellations or changes to daycare and other reservations.
Late Cancellations: Cancellations made with less than 1 days' notice may be subject to a cancellation fee equivalent to 25% of the estimated booking cost.
No Shows: Failure to show up for a scheduled reservation without prior notice (i.e., a "no-show") will result in a charge equivalent to 50% of the estimated booking cost (this means that your 50% deposit will not be refunded).
Refunds: Refunds for cancellations made within the specified notice period will be provided in the form of credit towards future daycare, boarding, or drop-in services or via the original payment method (Venmo, Zelle, etc).
Emergency Exceptions: I understand that emergencies can arise, and exceptions may be made on a case-by-case basis for documented emergencies or extenuating circumstances.
Holiday and Peak Periods: During holiday seasons or peak periods, such as major holidays or busy travel seasons, I may require a longer notice period for cancellations and may have different cancellation policies in place. Please inquire for specific details when booking during these times.
Reservation Modifications:
Changes to Reservation Dates: Changes to reservation dates made within the specified notice period are subject to availability and may incur additional fees.
Early Departures: Early departures or shortened stays must be communicated in advance and may be subject to fees equivalent to a portion of the original booking cost.
How to Cancel or Modify a Reservation:
To cancel or modify a reservation, please contact me directly by phone at (405) 825-1756 or email me at gibson.rae.jennifer@gmail.com at least 1 day prior to the scheduled reservation date. Be sure to provide your reservation details and any relevant information regarding the cancellation or modification request.
Policy Acceptance:
By booking daycare or other services with Sittermate LLC, you acknowledge and agree to abide by the terms and conditions outlined in this cancellation policy.
